Important Information – Please Read Carefully

• Submit one form per student. A new form is required for each additional student.
• Registration is only complete once you provide your electronic signature and pay the $25.00 non-refundable registration fee. (Note: Charter school funds cannot be used for this fee.)
• Once submitted, you may receive a follow-up phone call to confirm details. If approved, you’ll get a confirmation email regarding your student’s enrollment.
• We reserve the right to deny registration.
